You need to write your full name, date of birth, nationality, passport number, flight number, and your address in Japan. Please use block letters and write clearly. If you make a mistake, please let me know and I'll provide a new form.

You need to check yes or no for each question about items you are bringing into Japan. If you carry more than 1000000 JPY in cash, alcohol, tobacco, or gifts worth more than 200000 JPY, you must declare them. Please answer all questions honestly to avoid delays at customs.
