Thank you for coming on such short notice. We need to discuss the serious accident that occurred yesterday and the gaps in our emergency response system. Could you explain what happened and what immediate actions you have taken?
(急なご連絡にもかかわらずお越しいただきありがとうございます。昨日発生した重大事故と緊急対応体制の不備について話し合う必要があります。何が起こったのか、そしてどのような緊急措置を取ったのか説明していただけますか?)
🧑‍🎓【Student / Safety Manager】:
I sincerely apologize for this accident. A worker fell from scaffolding at 10 meters high because the safety harness was not properly attached. We immediately stopped all work at height, called an ambulance, and reported to local authorities within 2 hours. The worker is now in stable condition at the hospital.

The root cause is that our emergency contact list was outdated and the on-site team did not have clear authority to stop work immediately. We will update the emergency contact list every month and give all site supervisors the authority to stop work without approval. We will also set up a 24-hour emergency hotline starting next week.
